Garlic announces itself. Crush a clove into a hot pan and the whole kitchen turns toward the stove — the sizzle, the sweetness, the smell that gets into everything. In cooking that is the entire point. In a capsule you are meant to swallow with a meal twice a day, the smell and taste part somebody had to engineered away, but the benefits stay.
Which brings us to the bottle. Kyolic Formula 100 comes in a 300-capsule size that looks, sitting on a shelf next to the multivitamins, like a supply you will still be working through next spring. I've bought this bottle twice. The count on the front is real — it just answers a question nobody actually asked.
Capsules, servings, days: three different numbers
A serving here is two capsules, and two capsules provide 600 mg of aged garlic extract. So 300 capsules is 150 servings, which is where a lot of mental math stops. The label, however, directs adults to take two capsules with a meal, twice daily. That is two servings a day — four capsules, 1.2 grams of extract. [1,2]

Two and a half months, then. Worth knowing before you set it against a 100-count bottle of something taken once daily and decide which one is the bargain. Worth knowing too that the capsules contain whey (milk) and gelatin, which rules the product out for some people before any of the rest matters. [1,2]
What the aging is supposed to do
Wakunaga says its garlic is grown on certified organic farms in California and aged without heat for up to 20 months, a process it describes as converting harsh, unstable sulfur compounds into stable, odorless ones. [2] That is the company describing its own method — the useful thing to take from it is that "aged" here refers to a specific process, not a marketing adjective, and that the trials below tested extracts made that way.
What the trials measured
Blood pressure is where aged garlic extract has been studied most seriously. Systolic is the top number in a reading, diastolic the bottom one. Both trials below ran 12 weeks in adults with uncontrolled hypertension, and both used Kyolic Reserve — a different formulation from Formula 100, so the results below describe Reserve, not this bottle.
- AGE at Heart, 2016. Among 88 completers, systolic pressure fell 5.0 mmHg more than on placebo. [3]
- GarGIC, 2018. Among 49 completers, systolic fell 10 mmHg and diastolic 5.4 mmHg compared with placebo, and arterial stiffness — a secondary measure of how flexible blood vessels are — improved. [4]
Each used a daily dose supplying 1.2 grams of aged garlic extract along with 1.2 mg of S-allylcysteine (SAC), a compound formed during aging. Formula 100's label reports extract mass; the Reserve protocols specified SAC as well. Some participants were not on blood-pressure medication; those who were stayed on their established regimens. Wakunaga funded both trials, and both papers report that the sponsor had no role in design, data collection, analysis, or manuscript preparation. [3,4]
Widen the lens and the numbers shrink. A 2025 review pooling 19 randomized trials of various aged-garlic preparations found average reductions of 2.49 mmHg systolic and 4.41 mg/dL in LDL cholesterol, with no significant overall change in diastolic pressure or HDL. [5] Those are changes in measurements taken over a few months, which is a smaller claim than it first sounds like.
Before it goes in the cart
Skip it entirely if you are allergic to garlic or to milk. Garlic can increase bleeding risk, so talk with your clinician before starting if you take anticoagulants or aspirin or have surgery on the calendar — and keep taking whatever has been prescribed to you. [6]
